Podiatry services are available for all participants by special arrangement with Dr. Raymond T. Justice.

How do I enroll?
Schedule your appointment with the Nurse. Fill out and return a completed copy of the admission form for Dr. Justice. Medicare, Medicaid and any supplemental Insurance cards are required for the FIRST visit. Copies will be made of this information and kept on file for subsequent visits.

What is the cost?
Dr. Justice will bill services to Medicare/Medicaid or Supplemental Insurance.

When is the clinic?
The clinic is held approximately every sixty days. Watch the monthly Activity Calendar for exact dates.

Reminder: You may need to rearrange your attendance days if the Doctor is coming on a day you are not regularly scheduled. Speak to the Administrative Secretary, for all attendance changes.
